Healthy People, A Healthier America
PSW works with the Thurston County Health Department and the Washington State Department of Health to create a local model of the Washington State Diabetes Collaborative, based on Planned Care. Currently there are 6 practice sites and 7 providers participating in this project. Planning partners for this project are Northwest Physicians Network and St Peter Family Medicine.
This work began with Thurston County STEPS to a Healthier America Grant in 2005, and continues with an ASIST Healthy People 2010 Grant.
