Melanie Matthews, MSChief Executive Officer
Melanie Matthews is the dynamic, creative and innovative CEO at Physicians of Southwest Washington (PSW) and President at MultiCare Connected Care. She brings more than 20 years of operations, financial, human resources and product marketing experience in health care services for specialty populations. Her passion for public policy and engaging legislatures has propelled Melanie as the “voice” of physician health policies. She has a high personal capacity for energy, clear focus and performance excellence and is a highly-desired public speaker because of her nationally recognized visionary leadership.
Since she joined the company in 2016, she has maintained the core principals in which PSW was founded on and expanded business lines to include MSO services including credentialing, coding and compliance and the implementation of CMMI innovation models such as the Next Generation ACO. Melanie’s extensive knowledge in post-acute care provides strategic focus in reducing overall cost of care as well as provider and beneficiary engagement.
Prior to PSW, Melanie served for three years as Vice President of Operations for Prestige Care, Inc., where she was responsible for regulatory and financial operations and outcomes for 38 skilled nursing facilities and two Medicare home health agencies in a four-state northwest region. Among her other accomplishments, Melanie serves as Co-Chair for APG – Risk Evolution Taskforce, was selected by the American Health Care Association as a “National Political Ambassador” in 2013, and was named a national “Future Leader” in 2012.
Melanie holds a master of science, social gerontology, degree from Central Missouri State University and a bachelor’s degree in human development and family studies from Pennsylvania State University. She is married with two children and has lived in Olympia since 2008.

Tamara brings 22 years of experience focused on operational improvement, communications and human resource management. Responsible for the provider network, including contracting and credentialing, Tamara also oversees human resources, the development of communication campaigns and serves as the lead for business development initiatives.
Tamara began her health care career serving in a communications and operations role for a large independent imaging facility. Prior to healthcare, Tamara honed her leadership and operational skills in the non-profit sector for 12 years, serving as the director of two start-up – and still successful-non-profit organizations.
Tamara holds a Masters of Business Administration and a Lean Six Sigma Green Belt certification from Saint Martin’s University. Her professional accomplishments include chairing the Regional Advantage workshop series for Thurston and Lewis County; Chair of the inaugural Every Woman Counts Screening Mammography & Wellness Event; received Forty Under 40 Leadership Recognition and nominated as a Women of Influence.
Cheryl MosesChief Nursing Officer

Cheryl brings 30 years of experience in healthcare, quality improvement, and post-acute care. Responsible for oversight of
the Care Management Department and clinical quality improvement initiatives.
Cheryl began her career in healthcare in 1986 working in post-acute care setting. In 1994 she graduated from Yakima Valley Community College earning her Associate’s Degree in Nursing and later attended the University of Phoenix to earn her Bachelor’s and Master’s degrees in Science of Nursing.
Cheryl has held positions as a Regional Nurse Consultant for Prestige and Avalon Health Care. There she gained experience in developing and implementing quality improvement plans as well as submissions for recognition of outcomes through Qualis for Patient Safety and Baldrige Awards in post-acute care setting.

Tamra brings 25 years of experience in the payer setting with comprehensive experience in claims management, medical risk adjustment and population health analytics. Tamra oversees business intelligence, technology and clinical interfacing.
Prior to PSW, Tamra was a technical project manager at the Washington State Department of Labor & Industries, responsible for a biennial budget of 3.9M.
Tamra holds a Masters in Business Administration in Healthcare Management and serves on the South Puget Sound Community College committee for creating a curriculum to support medical billing and coding and is the Chair of the Oakville School district.
Kat KhachatourianChief Quality Officer
Kat Khachatourian, PharmD MBAChief Quality Officer
Dr. Khachatourian brings 20 years of healthcare experience to PSW as a residency trained managed care and ambulatory care pharmacist. Joining PSW as the Chief Quality Officer in 2019, she is responsible for Quality and Utilization Management program development in pursuit of improved quality of care and outcomes. Dr. Khachatourian completed her bachelor of science at Clayton State University in Atlanta, Georgia, where she also completed her Doctorate of pharmacy at Mercer University. After completing her managed care and ambulatory care residency at Group Health Cooperative in Seattle, Washington, she completed her masters in business administration at the University of Washington.
Prior to PSW, Dr. Khachatourian served for six years as Vice President of Strategic Care Transformation in the Medicare Advantage space, accountable for improvements in pharmacy clinical and Financial management, acute and non-acute medical management and population health, as well as strategic management of vendor compliance and administrative functions. Prior to and during pharmacy school, Dr. Khachatourian worked as a pharmacy technician and intern in community pharmacy practice for eleven years.
She also serves on the Professional Practice Committee for the Academy of Managed Care Pharmacy; the Cardio-Renal Board for Bayer Pharmaceuticals; and was appointed to the Washington State Pharmacy Commission in 2017. She is a managed care mentor for the University of Washington and Washington State University Schools of Pharmacy and also an active member of the Academy of Managed Care Pharmacy (AMCP) and Health Information Management Systems and Society (HIMSS).

John joined PSW in June 2019 as the Chief Information Officer. He is responsible for IT related activity and support in major PSW contracts.
John brings forty years in Healthcare IT experience, working in senior management IT roles on both the provider side for physician groups, Tenet Healthcare, Golden Living and Avalon Healthcare, and on the software product side for 3M Health Information Systems and Align Healthcare. John holds a Master of Public Health degree specializing in Health Information Systems from the University of Texas and an Executive Program certificate from the Anderson School at UCLA. He also holds a Six Sigma green belt.
Anchored in Wausau, Wisconsin, John travels to Washington monthly. A strong believer in volunteerism, he recently served in leadership positions for Kiwanis and the local council for the Boy Scouts of America.
Jessica Van Fleet-GreenChief Medical Officer

Dr. Van Fleet-Green is a board certified family physician. She joined Physicians of Southwest Washington (PSW) as a Medical Director in 2017 and in January 2019 transitioned into the role of Chief Medical Officer and interim Clinical Officer for MultiCare Connected Care. Dr. Van Fleet-Green has a strong interest in systems based practice and the development of value based metrics.
Dr. Van Fleet-Green completed her undergraduate education at the University of Washington where she also attended medical school. She completed her residency in Family Medicine through the University of Washington WWAMI program at Providence St. Peter Hospital in Olympia. Dr. Van Fleet-Green did full scope family medicine including obstetrics as a partner at Yelm Family Medicine, PLLC until April 2017 when she transitioned into clinical and administrative work in post-acute and long-term care. She continues her work as a Post-Acute and Long Term Care medical director at HCR ManorCare in Lacey, WA.
Dr. Van Fleet-Green is a fellow of the American Academy of Family Physicians and member of the Washington Academy of Family Physicians and has served on the Foundation Board as well as in the delegation. She is an active member of The Society for Post-Acute and Long-Term Care Medicine and is a Certified Medical Director.

Sara Aukerman serves as Compliance Officer for Physicians of Southwest Washington (PSW), NW Momentum Health Partners (NWMHP) and MultiCare Connected Care (MCC). In her 10 years at PSW, she is responsible for leading internal processes for ensuring compliance, mitigation, manages HIPAA privacy and security regulations, company policies and contracts. She is also responsible for formulating and implementing policies and procedures, while ensuring they are communicated and trained upon across all companies.
Sara joined PSW in 2009 in the administration department and worked her way through numerous divisions that spanned from claims to information technology. Prior to joining PSW, Sara worked in the medical industry and co-owned an event planning company.
Sara is certified as a professional coder and a professional compliance officer through the American Academy of Professional Coders (AAPC). She currently resides in Olympia, WA with her daughter and enjoys refinishing furniture and creating jewelry.

Kevin Haughton, MD, is a primary care physician in Olympia, WA and is certified in Family Medicine by the American Board of Family Medicine. Haughton serves as a Medical Director with Physicians of Southwest Washington, an Independent Practice Association managing Medicare Advantage and ACO contracts throughout the Northwest. Haughton graduated medical school at the University of California, Irvine, and has been practicing for over 30 years.

Michael Shannon, MDMedical Director
Michael H. Shannon, MD, is an endocrinologist and Medical Director with Physicians of Southwest Washington, an Independent Practice Association managing Medicare Advantage and ACO contracts, in Olympia, Washington. He is a Clinical Assistant Professor in the Department of Medicine at the University of Washington. He serves on the Editorial Board of Clinical Diabetes, and he is the Chair of the Professional Education Committee for the Washington chapter of the American Diabetes Association.
Shannon served as the System Endocrinology Lead and Chair of the Endocrine/Diabetes Clinical Advancement Team for Providence St. Joseph Health, a 51-hospital integrated health system, from 2011-2018. He served as the Medical Director for his region’s Accountable Care Network, dedicated to quality metrics and reductions in the total cost of care.
Dr. Shannon graduated from medical school at the University of California, San Diego, and then he went on to perform an internal medicine residency, geriatrics fellowship, and endocrinology fellowship at the University of Washington.
